- Player name: The nick name you were using when the ban was applied

your username is the word u write in the login when u connect to the servers (as shown in the picture below)

Before writing anything, remember that you are asking for a favor, then you should be nice. You can also see through reading old ban appeal threads that old stories of brother / friend using the account, etc. are not useful.
